Moving into aged care

Moving into residential aged care can be a big decision for both residents and family members. However, at Gracedale Home & Hospital, we are here to help. Gracedale offers Rest Home, Hospital and palliative level of care. A needs assessment from your GP will clarify the type of care that would be most suitable for your individual needs.
